Community Outreach – Health Care Engagement Specialist (EBCAP0146)

East Bay Community Action Program is seeking a full time Community Outreach – Healthcare Engagement Specialist for our Health Center in Newport, RI. The Community Outreach-Healthcare Engagement Specialist will provide a wide array of culturally competent, recovery oriented duties to help clients/patients connect with their care team. The ideal candidate will work with the care teams to identify clients/patients needing integrated health services. Outreach may be needed in the home, emergency departments, inpatient discharge in a hospital, community center or other venues where the client/patient may be located. The population served includes adults and families needing services for integrated health services, including behavioral health/substance abuse, primary care, and basic services to improve the quality of their lives. In addition, works closely with clinicians, case managers, nurses and medical providers and health advocates to engage and re-engage clients in services. The ideal candidate requires knowledge of the community and confidence working with at-risk populations, including those with a chronic disease, serious behavioral health diagnosis and/or those reluctant to receive healthcare services. The position includes flexible hours; days, evenings and weekends to provide services to patients/clients.

Job Responsibilities:

  • Provide individual outreach, as needed, to help clients/patients initially engage or maintain engagement in services
  • Provide education and alternatives to enable clients/patients to access services independently
  • Complete accurate documentation according to agency policy and timelines.


  • High School or College.
  • Commitment to improving access to integrated healthcare •Ability to communicate well and maintain respect for a diverse population of clients/patients.
  • Ability to work with an interdisciplinary care team.
  • Ability to work independently and remain organized and efficient while multitasking.
  • Healthcare or behavioral health experience helpful, however, not required.
  • Veteran or law enforcement or security background helpful/not required.
  • Bilingual (Spanish/English) preferred, however, not required.
  • Must have car for outreach in the community.
  • Superior ethics as evidenced by past experience and community based work.
  • The position includes flexible hours; days, evenings and weekends to provide services to patients/clients.
