Case Manager - Behavioral Health (EBCAP0086)

East Bay Center a division of East Bay Community Action Program is seeking a full time (40 hours) Case Manager for Behavioral Services at our location in Barrington, RI. The Case manager will coordinate services for clients to minimize stressors and to facilitate the reduction of psychiatric and substance abuse disorder symptoms. In addition, encourage clients to obtain the highest possible levels of independence. Care and treatment of clients is provided within a multidisciplinary treatment team.

Job Responsibilities:

· Provide advocacy and/or act as a community liaison in all aspects of clients’ lives in the community, including, but not limited to, working with property owners, families, police, healthcare providers, and judicial system personnel, social service providers, and other community support systems designed to ensure the clients’ independence in the community.

· Maintain a working knowledge within scope of position of specific State, Team, JCAHO, and Medicaid/BHDDH guidelines. Assist the team in preparing for and/or facilitating BHDDH Certification Site visits, as well as maintaining ongoing Medicaid reimbursement ability by ensuring ream compliance with standards.

· Participate as a member of a multidisciplinary team in ongoing quality improvement efforts, treatment planning meetings, conferring with Center staff on individual cases, and promoting collaboration efforts within case management. Participate in organizational/case review meeting with team members.

Qualifications:

  • Bachelors’ degree in a Human Services field required and one year of previous Human Services experience preferred.
  • Must have a valid driver’s license/automobile insurance due to the amount of community travel.
  • Bilingual candidates (Portuguese, American Sign Language, etc.) are encouraged to apply.

  • Ability to interpret documents (i.e., policy and procedure manuals) and write reports and correspondence pertaining to clients.
