Description: The family advocates provide community-based case management and assess, implement and maintain a care plan according to the client's needs and choices in compliance with The Department of Elderly Affairs Home and Community Care Program. The goal is to provide the choice between nursing home and independent living with the implementation of services in the home to enable the client to remain independent and safe. This also includes the choice of assisted living on a special waiver program.
Family advocates also assess for interventions for the State of RI Protective Services, Early Intervention and Self-Neglect Programs. These interventions attempted with the most resistive clients who are not looking for change could include the above home care management, but more often will include bringing food, coordinating a major house cleaning, a physician's visit, a hospitalization, facilitating a family conference or a mental health or local police department intervention.
In addition, our RN/Family advocate provides ongoing support for clients that have issues surrounding the caregiver, providing both individual and group supports.
Eligibility Requirements-Program eligibility is 65 years of age or older. Other requirements vary for different programs and we work with individuals whose income ranges from Medicaid to no limits to assure they get the correct information for whatever their needs require. For some programs a requirement is that the customer needs and will accept personal care.
Catchment Area Served- Barrington, Bristol, East Providence, Warren and Pawtucket/Central Falls.
Ocean State Senior Dining Program
The Ocean State Senior Dining Program makes hot, nutritious meals available Monday through Friday to persons 60 years of age and older and/or handicapped persons.
Dining sites are located in East Providence, Barrington, Warren, Bristol, Tiverton, Portsmouth, Middletown, and Newport. Click here for a list of dining sites. Meals are served in the dining rooms, providing a social setting for mealtime and senior activities such as knitting, quilting, cards, keno, pool, exercise programs, bingo, special luncheons and trips. A donation of $3.00 is suggested, but not required. Reservations are required at least 24 hours in advance.
Soup to Go
A soup program began at the Middletown Senior Center and the East Providence Senior Center on Waterman Avenue on October 14, 2009. Anyone interested can purchase a pint for $2.00 or a quart for $4.00 by contacting the Middletown Senior Center at 401-849-8823 or the East Providence Senior Center at 401-435-7872. |
